ALASKA The Online Examination may be open book with no monitor. Students must sign The Affidavit of Student - Open Book. Print, complete and return this Affidavit and The Course Evaluation Form to:

CD-ROM students are additionally required to submit the course quiz answer sheets which are tabulated at the end of each chapter of study.
