Requirements for Online Final Examination

The Online Examination is closed book and must be monitored by a New York State approved Monitor. A list of approved monitors can be found at www.ins.state.ny.us. The monitor must sign The Affidavit of Student and New York Approved Examination Monitor and return this with a copy of their New York Monitor Approval Document to Medical Directions, Inc. The student must return The Course Evaluation Form to:
