CALIFORNIA The Online Examination may be open book with no monitor. Students must sign The Affidavit of Student - Open Book. Print, complete and return this Affidavit and The Course Evaluation Form to:

Students should confirm their employer's acceptance of the course prior to ordering as there is a NO REFUND policy for Online Training. CD-ROM students are additionally required to submit the course quiz answer sheets completed at the end of each chapter of the CD-ROM training program. IMPORTANT: Before beginning your examination read the specific state requirements for online testing and print any required documents. Definitions of disinterested third parties vary from state to state. Suggestions for disinterested third parties include librarians, CPAs, attorneys or clergy.
